Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.

VRS Social Worker Assistant/Case Manager

UBA WeCARE provides a continuum of services to help HRA referred cash assistance clients with medical and/or mental health conditions that affect their employability to attain their maximum levels of self-sufficiency. Individuals referred to WeCARE receive medical and psychosocial assessments to identify possible clinical conditions and social barriers that may affect their ability to participate in work-related activities. Based on the results of the assessments, a determination of an individual's functional capacity outcome (FCO) is completed, and a customized service plan is developed to provide a range of services tailored to meet a client's particular needs.

We are currently seeking a VRS Social Worker Assistant/Case Manager (CM)

to join the UBA WeCARE program.

Job Responsibilities:

  • The CM is responsible for providing services to WeCARE clients who are deemed to be employable with accommodations. 
  • The CM implements the service plan with a focus on supporting clients towards employment while reducing barriers to employment.
  • The CM will also provide outreach to remind clients of pending appointments and attempt to re-engage clients if they have missed an appointment.  

Job Duties:

  • Thoroughly review clients' clinical documents (current and previously provided), that support client's employability.
  • Complete VRS Service Initiations and maximize clients' engagement. 
  • Implement the service plans with the clients.
  • Manage assigned activities and caseloads.
  • Make appropriate childcare referrals.
  • Provide Enhanced Case Management services as needed.
  • Track and document work activity.
  • Identify clients' need for reasonable accommodations.
  • Use skills to successfully engage clients and diffuse tension so that they participate in the evaluation process.

  • Bachelor’s Degree required.
  • Master’s Degree preferred.
  • Length of Experience Required:  3 Years

Training Required:  Knowledge of vocational rehabilitation
